Telehealth Consent

Draft for attorney review. Not yet approved.

Last updated: November 5, 2026

Pure Wellness Medical provides care by telehealth. This page explains how video visits work, what they can and cannot do, and when you may need to be seen in person. Please read it before your first visit.

How video visits work

  • Your visits with Dr. Suarez, including your consultation, results consult and scheduled reviews, happen on live video. Your coach and care coordinator support you between visits.
  • You join from your phone, tablet or computer. You need a camera, a microphone and a reliable internet connection. Please join from a private place where you can speak freely.
  • At each visit, you need to be in a state where we provide care: every state except California and Nevada. We may ask you to confirm who you are and where you are at the start of a visit.
  • Blood draws, imaging, DEXA, infusions and scans are arranged at partner facilities near your home. Your results come to Dr. Suarez, who reviews them with you.
  • When a prescription is clinically appropriate, it is sent to a licensed U.S. pharmacy and delivered to your door.
  • Your visits are documented in your medical record, the same as an office visit, and your information is protected under health privacy laws.

The limits of telehealth

  • Dr. Suarez cannot touch, listen to or physically examine you by video. Some problems can only be found in person, so a video visit may miss something an office exam would catch.
  • Technology can fail. If the video or sound drops, we will try to reconnect, and if we cannot, we will reschedule.
  • We use safeguards to protect your information, but any electronic communication carries a small risk of being intercepted or accessed without permission.
  • Telehealth is not for emergencies. If you have a medical emergency, call 911.
  • Our care does not replace your primary care doctor. Keep your primary care doctor for routine and sick visits.

When an in-person visit may be needed

Dr. Suarez will tell you if your care needs an in-person visit, and we will help arrange it near your home. This can happen when:

  • He needs a physical exam or a hands-on test to make a safe decision about your treatment.
  • A medication you need is a controlled substance. Some prescriptions, like testosterone, are controlled substances, and federal rules can require an in-person visit before we start them; if that applies to you, we’ll tell you at your consultation and arrange that visit before you start.
  • Your labs or imaging show something that needs a closer look. When they do, we coordinate the specialist referral and follow-up.
  • Your symptoms change or get worse in a way that video cannot safely assess.

Your part

  • Give us a complete and honest history, including every medication and supplement you take.
  • Tell us where you are at the time of each visit.
  • Complete the labs, imaging and follow-up visits Dr. Suarez orders.
  • Tell us about other doctors you see, so your care can be coordinated.

Your rights

  • You can ask questions about telehealth at any time, before or during a visit.
  • You can withdraw this consent at any time by telling us in writing. Because Pure Wellness Medical provides care by telehealth, withdrawing it may mean we can no longer treat you; if so, we will help you find in-person care.
  • You can ask for a copy of your medical records.

Your agreement

By agreeing to this consent, you confirm that you have read it, that you have had the chance to ask questions, and that you agree to receive care by telehealth from Dr. David Suarez and Pure Wellness Medical.
